People are living longer and with more chronic diseases such as asthma, diabetes and heart disease. In addition, they often deal with problems such as pain, fatigue, frustration and isolation. They have multiple providers, treatments and medications and may find it difficult to communicate their needs to their physicians and family members. Many times, they do not know the appropriate exercise and nutrition that will help them maintain a healthy level of functioning. These difficulties can often lead to deterioration of their condition and increased visits to the emergency room or hospital admissions.
The Care New England Wellness Center's Living Well Program provides individuals living with chronic disease the tools they need to self-manage their condition. The program is a six-week program taught by trained staff that teaches patients with chronic disease how to better manage their symptoms, follow medication regimens, and manage functional ability to improve quality of life. It is an evidence-based program developed by Stanford University and is endorsed by the Rhode Island Department of Health, Rhode Island Department of Human Services, and Rhode Island Department of Elderly Affairs.
